- Previous last date: 30 April 2025
- New last date: 15 May 2025
This extension allows more families to apply through the self-survey method.
📲 Step-by-Step: How to Fill PMAYG Self Survey Online Form 2025
Step 1: Download the Required Apps
- Install “Awas Plus” app from Google Play Store (published by the Department of Rural Development).
- Also install “Aadhaar Face RD” app for Aadhaar-based face authentication.
Step 2: Begin the Self-Survey
- Open Awas Plus App → Select “Self Survey” option.
- Enter Aadhaar number and complete face authentication (remove cap/sunglasses, good lighting, blink eyes).
Step 3: Enter Your Location
- Choose your State, District, Block, Panchayat, and Village.
Step 4: Add Survey Details
- Go to Add AD Survey section.
- Fill in:
- Name of applicant
- Family head name
- Aadhaar number
- Mobile number
- Age, gender, marital status
- Social category (SC/ST/OBC/General)
- Occupation (labourer, vendor, farmer, etc.)
- Literacy status
- Annual family income (no income certificate needed)

- Enter details of each family member (Aadhaar, age, relation, occupation, mobile).
- You cannot proceed unless all members are added.
👩🦰 Priority to Women Applicants
- Select a woman (wife, daughter) in the family as the main beneficiary.
- If no woman exists, male application is allowed.
🏦 Enter Bank Details
- Choose your bank/post office → Branch name → Enter account number.
- If no bank account, select “I don’t have a bank account” (can update later).

- Your name will be added to the beneficiary list.
